Reservation & Cancellation Policy
As a courtesy to our medical spa professionals, please provide a minimum of a 24-hour notice should you need to cancel or reschedule an appointment. Appointments scheduled for 1.5 hours or longer require a minimum of a 48-hour notice for any cancellations or rescheduling.
A credit card is required at the time of booking. You may be charged a deposit/cancellation fee ranging from $50 to $200 for appointments canceled outside of the required notice window or in the event of a no-show. The fee amount is dependent on the value and duration of the service booked. For certain premium injectable services lasting 1.5 hours or more, a deposit fee ranging from $150 to $200 is required at the time of booking to secure your appointment.
If you attend your scheduled treatment, the deposit will be applied to your treatment cost on that day.
Less than 48 hours prior to appointments 1.5 hours or longer, the deposit will be forfeited.

Late Policy
We cannot guarantee your appointment if you are more than 10 minutes late. It is our recommendation to arrive at least 15 minutes prior to your treatment time as our scheduling is designed to allow the correct amount of time needed to complete your experience. If you are running late, please call us so we may try to accommodate you; however, we cannot guarantee the full length or guarantee of the service.
Return Policy
In-Store Service: Once an in-store service has been rendered, it is non-refundable. Due to the nature of our services, refunds are not available once the service has been completed.
Packages: Refunds for service packages are based on unused treatments only. If all treatments have been completed, no refund is due, including in the event of an adverse reaction. All refund prorating is calculated based on unused treatments.
If any treatments have been redeemed from a promotional or discounted package, those services will be repriced at full value at the time of refund calculation, and the remaining balance will be adjusted accordingly.
Refund requests made within 30 days of executing this Agreement may be issued as a refund to the original form of payment or as store credit. Requests made after 30 days will be issued as store credit only.
Payments must be current to receive services. Store credit may be used toward any Revive services, packages, or products.
If a financing partner was used, refunds requested after 30 days will be subject to a 6% adjustment to account for financing fees incurred at the time of the original transaction.
Products & Gift Cards: If you are not completely satisfied with your purchase, simply return the item in its original packaging within 14 days for store credit only. We do not accept returns of SkinMedica, Latisse, Upneeq, or other prescription products. We do not accept returns on gift cards.
Payments
We accept all major credit cards and cash. All prices are subject to change without notice.
Gift Cards
Gift cards do not expire in client’s account and can be used on any service, product, or package purchase. Gift cards cannot be used on any iThrive wellness services.
Gratuities
Gratuities are not included in service price and are a matter of personal discretion on all of our services.
Pets
No pets please; service animals are welcome.
Revive Radiance Reserve Membership Terms and Automatic Renewal Notice
Radiance Reserve Membership can be purchased at any time during the year and is valid for one year from the date the membership is purchased or successfully renewed. Revive will automatically charge your payment method $199.00 every year until you cancel. Your annual rate is subject to change, but Revive will notify you of any rate change at least fifteen (15) days before your renewal date so that you have the opportunity to cancel before the new rate is charged. You must cancel your subscription before the annual renewal to avoid the renewal charge. You may cancel your membership at any time online by logging into your user profile at store.revive.md or by contacting Revive directly via email at info@revive.md or via phone at 747.298.5327.
Memberships are non-refundable. If you cancel your membership (i.e., elect not to renew for the next annual term), your membership will not renew, but you will continue to have access to your membership benefits, including any unused membership treatment credit and membership discounts, through the remainder of your current 12-month membership term. Revive is not responsible for any overdraft or additional fees related to subscription charges.
Radiance Reserve Membership includes one membership treatment credit per 12-month membership period. The membership treatment credit may be redeemed for one of the following: 15 units of Botox or Daxxify, 45 units of Dysport, one small or medium laser hair removal (LHR) area, or one exosome or peptide add-on. Only one item may be selected per membership treatment credit, and the credit may not be split or partially redeemed across more than one item or service. The membership treatment credit may be redeemed at any time during the applicable 12-month membership period. Membership treatment credits and membership discounts are non-transferable and may only be used by the active member. The membership treatment credit is a personal benefit of an active Radiance Reserve Membership, not a purchased or gifted certificate, and is not separately sold, assigned, or transferred to any other person. The membership treatment credit has no cash value and may not be redeemed for cash, credit, or any other form of compensation. Unused membership treatment credits expire at the end of the 12-month membership period and cannot be rolled over or carried forward into a subsequent membership period. A new membership treatment credit will be provided upon each successful annual membership renewal.
Membership discounts are applicable only to services indicated and exclude all other services. No minimum purchase is necessary to join Radiance Reserve Membership. The membership fee cannot be applied toward any services, products, or gift card purchases. Membership is non-transferable. Membership discounts cannot be applied to past purchases. Membership discounts have no monetary value and cannot be exchanged for cash or credit. Membership discounts cannot be combined with any other promotions except where explicitly stated. Additional Membership Terms
Eligibility. You must be at least eighteen (18) years of age to purchase or hold a Radiance Reserve Membership. Membership is available only to individuals; business entities may not enroll. Redemption of the membership treatment credit is subject to completion of any required patient intake, clinical evaluation, and informed consent as determined by the treating provider. Revive reserves the right to decline membership enrollment or treatment credit redemption to any individual who does not meet applicable eligibility or clinical suitability requirements.
Assumption of Risk; Informed Consent. You acknowledge that the treatments available through the Radiance Reserve Membership, including injectables and laser procedures, are medical aesthetic procedures that carry inherent risks, including but not limited to bruising, swelling, infection, scarring, allergic reaction, and unsatisfactory results. All treatments are performed by licensed healthcare professionals and are subject to a separate informed consent provided at the time of service. Your acceptance of these Membership Terms does not constitute informed consent for any specific treatment. You agree that Revive's liability for any treatment-related claim is governed solely by the applicable informed consent and professional standard of care, not by these Membership Terms.
Service Availability. Revive reserves the right to modify, substitute, or discontinue any treatment included in the membership treatment credit menu or any service eligible for a membership discount, at any time and for any reason, including product unavailability, changes in clinical guidelines, or provider availability. If a listed treatment becomes unavailable, Revive will offer a comparable substitute treatment at its discretion. Appointment availability is subject to scheduling capacity, and Revive does not guarantee that appointments will be available on any particular date or time.
Appointment Cancellations and No-Shows. Members are expected to provide at least twenty-four (24) hours' notice when canceling or rescheduling a treatment appointment. Revive reserves the right to charge a cancellation or no-show fee for missed appointments or late cancellations, in accordance with its then-current cancellation policy. Repeated no-shows may result in restrictions on future scheduling.
Limitation of Liability. To the fullest extent permitted by applicable law, Revive's total aggregate liability to you arising out of or relating to these Membership Terms or the Radiance Reserve Membership, whether in contract, tort, or otherwise, shall not exceed the total membership fees actually paid by you during the twelve (12) month membership period in which the claim arose. In no event shall Revive be liable for any indirect, incidental, special, consequential, or punitive damages, including but not limited to loss of profits, data, or goodwill, regardless of the theory of liability. Nothing in this section limits or excludes liability that cannot be limited or excluded under applicable law, including liability for gross negligence, willful misconduct, or fraud.
Modification of Terms. Revive reserves the right to modify these Membership Terms at any time. Updated terms will be posted at store.revive.md, and your continued membership after the effective date of any modification constitutes your acceptance of the updated terms. Material changes will be communicated to you via email to the address on file at least fifteen (15) days before they take effect. If you do not agree to the modified terms, your sole remedy is to cancel your membership before the changes take effect.
Privacy. Your personal information, including payment details, contact information, and treatment history, is collected, used, and protected in accordance with Revive's Privacy Policy, available on our website. By enrolling in the Radiance Reserve Membership, you consent to the collection and use of your information as described in the Privacy Policy, which is incorporated into these Membership Terms by reference.
Force Majeure; Suspension of Services. If Revive is unable to provide services due to circumstances beyond its reasonable control, including natural disasters, pandemics, government orders, utility failures, or supply shortages, Revive may temporarily suspend membership benefits for the duration of the disruption. Any membership treatment credit that would otherwise expire during a suspension period will be extended for a period equal to the duration of the suspension. Revive shall not be liable for any failure or delay in performance resulting from such circumstances.
Electronic Communications and Consent. By enrolling in the Radiance Reserve Membership, you consent to receive communications from Revive electronically, including membership confirmations, renewal notices, rate change notifications, and modifications to these Membership Terms. You agree that all agreements, notices, and other communications provided electronically satisfy any legal requirement that such communications be in writing. You may withdraw your consent to receive electronic communications by canceling your membership.
Notices. All notices from Revive to you will be sent to the email address associated with your membership account and will be deemed received on the date sent. It is your responsibility to maintain a current and valid email address on file. Notices from you to Revive must be sent to info@revive.md or to such other address as Revive may designate.
Entire Agreement. These Membership Terms, together with any informed consent executed at the time of treatment and Revive's Privacy Policy, constitute the entire agreement between you and Revive with respect to the Radiance Reserve Membership and supersede all prior or contemporaneous oral or written agreements, representations, or understandings relating to the subject matter hereof.
Waiver. No failure or delay by Revive in exercising any right, remedy, or privilege under these Membership Terms shall operate as a waiver thereof, nor shall any single or partial exercise of any right, remedy, or privilege preclude any further exercise thereof or the exercise of any other right, remedy, or privilege.
Governing Law; Dispute Resolution
These Membership Terms and any dispute arising out of or relating to these Membership Terms or the Radiance Reserve Membership, including any question regarding its existence, validity, or termination, shall be governed by and construed in accordance with the laws of the State of California, without regard to its conflict-of-laws principles.
Any dispute, claim, or controversy arising out of or relating to these Membership Terms or the breach, termination, enforcement, interpretation, or validity thereof, including the determination of the scope or applicability of this agreement to arbitrate, shall be determined by binding arbitration administered by JAMS pursuant to its Streamlined Arbitration Rules and Procedures then in effect. The arbitration shall take place in Los Angeles County, California, before a single arbitrator. Judgment on the arbitration award may be entered in any court having jurisdiction. Each party shall bear its own attorneys' fees and costs in connection with the arbitration unless the arbitrator determines that a claim or defense was frivolous, in which case the arbitrator may award reasonable attorneys' fees and costs to the prevailing party.
To the fullest extent permitted by applicable law, you and Revive each waive the right to a jury trial and the right to participate in a class action, collective action, private attorney general action, or other representative proceeding of any kind. All claims must be brought in the parties' individual capacity and not as a plaintiff or class member in any purported class, collective, or representative proceeding.
Notwithstanding the foregoing, either party may seek injunctive or other equitable relief in any court of competent jurisdiction to prevent the actual or threatened infringement, misappropriation, or violation of a party's intellectual property rights or confidential information.
If any portion of this Governing Law; Dispute Resolution section is found to be unenforceable, the remaining portions shall remain in full force and effect. If the class action waiver is found to be unenforceable as to a particular claim or request for relief, then that claim or request for relief shall be severed and proceeded with in a court of competent jurisdiction in Los Angeles County, California, while the remaining claims shall continue in arbitration.
